Care Management and Service Coordination

At Parkland Community Health Plan, care management is a collaborative process between our team, our providers, and our members. Together, we assess, plan, implement, coordinate, and monitor services to meet each member’s health needs—helping ensure access to quality, cost-effective care and improved outcomes.

Our Service Coordination team supports members with special health care needs, including those managing complex or chronic conditions. Using communication and available resources, we help members navigate their care efficiently, avoid unnecessary medical costs, and address non-medical drivers of health such as transportation, housing, or food access. Members who may benefit from additional education, care coordination, or assistance are identified through utilization management activities, health risk assessments, and administrative data reviews.

Through proactive outreach calls, we engage members about joining our service coordination program. Once enrolled, we complete a health needs screening and partner with the member and their provider to develop a personalized service (care) plan. Copies are shared with both the member and provider, and our coordinators follow up regularly to review progress, update goals, and adjust support as needs change. Members and providers may also contact our Service Coordination team directly for assistance or referrals.


Condition Management Guidance

Parkland Community Health Plan offers specialized condition management programs for asthma and diabetes. These programs provide education, coaching, and tools that help members take an active role in managing their health.

Our service coordinators work one-on-one with members to complete health risk assessments, create action plans, and coordinate services such as specialty care, equipment, or behavioral health support. We focus on removing barriers to care, encouraging adherence to treatment plans, and promoting lifestyle changes and self-management skills that lead to better long-term health.

Through collaboration with our provider partners, we ensure that members receive consistent, coordinated care across all programs—empowering healthier outcomes for the people and families we serve.
